亚低温对脑梗死IL-6和sICAM-1表达的影响 被引量:14

Effect of mild hypothermia on the expressions of IL-6 and ICAM-1 after MCAO in cerebral infarction

摘要 目的研究亚低温对脑梗死IL-6和sICAM-1表达的影响,探讨亚低温对脑梗死损害的神经保护作用机制。方法发病24 h内入院的脑梗死患者40例,随机分为亚低温组和对照组,各20例。分别置于冰毯机和常温床上,使其肛温保持在(33±1)℃和(37±0.5)℃。2组患者分别于入院后1、37、d抽血,应用酶联免疫吸附法(ELISA)检测血清IL-6和sICAM水平。于入院后当天及第30天时行神经功能缺损评分。结果亚低温组患者血清IL-6和sICAM-1水平在发病不同时间内均低于对照组,血清IL-6和ICAM-1水平随时间递减;亚低温组神经功能缺损评分在第30天时明显低于对照组。亚低温组治疗期间未发生严重并发症。结论亚低温可降低脑梗死的IL-6和sICAM-1的表达,改善发病第30天的预后,推测亚低温降低IL-6和sICAM-1的表达为亚低温减轻脑缺性损害的神经保护作用机制之一。 Objective To investigate whether mild hypothermia is associated with the expression of IL-6 and sICAM-1 in patients with massive hemispheric infarction,and explore the neuroprotective mechanism of mild hypothermia treating ischemic stroke.Methods All 40 patients with massive hemispheric infarction were enrolled,and were rondomly divided into two groups: the mild hypothermia group and control group.The mild hypothermia group and control group were treated with ice-blanket machine and normal bed,respectively.The rectal temperature of experiment group was controlled at(33±1)℃ the control group was controlled at(37±0.5)℃.The blood samples were drawed at the 1st,3rd,7th day after admittion.The serum levels of IL-6 and sICAM-1 were tested.Results IL-6 and sICAM-1serum levels on the lst,3rd and 7th day in the mild hypothermia group were lower than that of controls,IL-6 and ICAM-1 levels gradually decreased in different times.The degree of neurological deficits on 30th day in the mild hypothermia group was lower than that of the controls.And there was no severe complication during hypothemic therapy.Conclusion Mild hypothermia can reduce the expressions of IL-6 and sICAM-1 after cerebral infarction and improve the prognosis of cerebral infarction,so we presume that reducing the expression of IL-6 and ICAM-1 is one of the neuroprotective mechanisms in treating ischemic stroke by using mild hypothermia.
