ARDS患者俯卧位机械通气的效果观察和护理

Research on Effect Observation and Nursing of Prone Position Mechanical Ventilation for ARDS Patients

目的探讨俯卧位机械通气对ARDS患者氧合状态的影响。方法选择ARDS机械通气患者16例,采取俯卧位2h,1～2次/d。分别观察俯卧位后5,30,60,120min4个不同时段患者HR、MAP、SpO2、PaO2较俯卧位前5min的变化。结果俯卧位机械通气对患者HR、MAP无显著改变(P>0.05);俯卧位后5min患者SpO2无明显改变(P>0.05);30,60,120min患者SpO2明显改善(P<0.05)。俯卧位后60,120min患者的PaO2较俯卧位前明显改善(P<0.05)。结论俯卧位机械通气能改善ARDS患者氧合状态,可做为ARDS患者辅助治疗措施之一。

Objective To probe into the effects of prone position mechanical ventilation on oxygenation of ARDS patients. Methods To select 16 mechanical ventilation ARDS patients, who have prone position for 2 hours, 1-2 times a day. To examine each patient＇ s different conditions of HR, MAP, SpO2 and PaO2 when they take prone position during different period for 5,30,60 or 120 minutes,when compared with the conditions without having prone position. Results Prone position mechanical ventilation has no obvious effects on patient＇s HR and MAP（P 〉 0.05） ;As for 5 minutes＇ prone position, patient＇s SpO2 has no obvious effects （ P 〉 0.05 ） ; As for 30,60 or 120 minutes＇ prone position, patient＇ s SpO2 has improves obviously （ P 〈 0.05 ） ; As for 60 or 120 minutes＇ prone position, patient＇ s SpO2 improves obviously when compared with the conditions without having prone position（P 〈0.05）. Conclusion Prone position mechanical ventilation can improve ARDS patients＇ oxygenation, which may become one of auxiliary treatments for ARDS patients.
