脑干卒中26例临床分析

Management of patients with brainstem stroke:report of 26 cases

目的:探讨脑干卒中患者的救治及转归。方法:回顾分析我院自2004年5月～2005年4月收治的26例脑干卒中患者的临床资料。患者接受颅脑CT或MRI扫描确诊。其中9例脑干出血,17例脑梗死;10例合并严重并发症患者进入重症监护病房(ICU)接受治疗。结果:26例患者平均64.6(42～79)岁,病程平均(14.3±3.6)h。经治疗,5例痊愈,好转11例,死亡10例。随访11例平均17个月,其中合并语言障碍5例,呑咽困难4例,四肢功能障碍7例。结论:脑干卒中极易出现中枢性呼吸、循环功能衰竭或周围性呼吸功能衰竭,病死率高,需重视早期诊断及救治。

Objective ：To analyze the treatment and results of brainstem stroke in our department.Methods：We retrospectively reviewed the records of 26 patients diagnosed and confirmed with brainstem stroke at our center from May 2004 to April 2005.The cranial CT or MRI scan was performed and specific therapy （e.g,heparin）was initiated.Among them,9 patients had brainstem hemorrhage and 17 brainstem infarction.Ten patients with severe complications accepted the treatment in the intensive care units （ICUs）.Results ：The mean age of the patients was 64.6 years （range 42 to 79）.The mean time ranged from the onset of cerebral symptoms to the time of hospital admission was （14.3±3.6）h.Five patients were cured and 11 improved, 10 died.Followup was available on 11 patients （mean17 months） with moderate to severe functional disabilities including language barrier in 5, deglutition disorders in 4,limbs disability in 7.Conclusion ：The patients with brainstem stroke may develop central respiratory or circulatory dysfunction in the early time,have a high mortality,need timely diagnosis and treatment.
