腰椎后路椎间盘镜治疗腰椎间盘突出症82例疗效分析

Clinical outcomes of intervertebral disc endoscopic therapy through posterior approach for lumbar intervertebral disc protrusion

目的:探讨后路椎间盘镜治疗腰椎间盘突出症(LDH)的手术技巧及其手术并发症的防治。方法:应用显微外科技术对82例LDH病人行手术治疗,术后采用Macnab标准评估疗效。结果:82例均随访3个月～1年,平均为5个月。疗效优69例(84%),良11例(13.4%),差2例(2.4%),优良率97.6%。术中出血60～250ml,平均120ml。手术时间50min^3h。结论:腰椎后路椎间盘镜手术减压充分、适应证宽、创伤小、出血少、术后恢复快。

Objective：To analyze the technical details and complications of intervertebral discs endoscope through posterior approach for lumbar intervertebral disc protrusion.Methods：A total of 80 patients received microsurgical treatment for prolased lumbar intervertebraI disc.The outcomes were evaluated according to Macnab standard.Results：All the 80 patients were followed up from 3 months to 2 years, averaged 5 years,the clinical results were rated as excellent in 69（84%）,good in 11（13.4%）,and poor in 2（2.4%）.The excellent and good rate was 97.6%.The blood loss varied from 60 to 250 mL with an average of 120 mL.The operative time ranged from 50 min to 3 h. Conclusion：The procedure has more indications and some advantages of minimal invasion,less bleeding, high security and quicker recovery on the premise of complete decompression to spinal cord and nerve root.
