2009年度心律失常亮点概述 被引量：4

摘要 盘点2009年,心律失常领域亮点纷呈:①Record-AF注册研究显示心房颤动(简称房颤)室率与节律控制疗效相当。②APAF2、ThermoCoolAF等研究进一步验证了导管消融治疗房颤的有效性。③RE-LY研究证实达比加群在房颤抗凝中安全有效。④ATHENA试验显示决奈达隆可显著降低无心力衰竭(简称心衰)或伴轻度心衰房颤患者全因心血管住院率或死亡率,但ANDROMEDA研究显示决奈达隆增加中重度左心衰竭患者死亡率。⑤PASCAI研究显示胺碘酮类似物budiodarone安全有效,且疗效呈剂量依赖性。⑥ACTIVEⅠ研究显示厄贝沙坦能显著降低房颤患者因心衰住院率、脑卒中和短暂性脑缺血发作。⑦ALTITUDE、MADITⅡ研究再次证实了埋藏式心脏转复除颤器(ICD)和心脏再同步化治疗和除颤器(CRT-D)对生存率的有益作用,但不恰当放电占43%,放电是死亡率的高危因子。⑧MEDIT-CRT研究显示对于轻度心衰症状、射血分数降低和宽QRS波心衰患者中植入CRT-D可进一步降低死亡率和心衰事件发生率。⑨抗电磁性起搏器及皮下ICD的问世。⑩对于高频率的室性期前收缩伴心功能略有降低患者,如果无其他心衰病因,可采用药物或射频消融治疗,此首次被专家共识正式列为消融治疗适应证。
