摘要 目的测量比较全膝关节置换术中确定股骨假体旋转对线的参照方法—股骨上髁轴线与Whiteside's线的准确性,探讨其在人工膝关节置换术中的应用。方法80例正常成人膝关节,通过Mimics10.01软件于膝关节横断面中测量股骨上髁轴线与股骨后髁轴线之间的夹角及Whiteside's线的垂线与股骨后髁轴线的夹角,运用统计学方法分析两角度间及其在性别间的差异性。结果PCA:男性3.56°±1.27°(1.42~6.65°),女性3.45°±1.09°(1.10~5.65°);PAPA:男性3.40°±1.60°(-1.01~6.06°),女性3.99°±2.06°(-0.84~7.76°)。PCA与PAPA的差异无统计学意义,性别间PCA及PAPA差异均无统计学意义,PAPA有较大的个体差异。结论全膝关节置换术中选择股骨上髁轴线或Whiteside's线做为参照轴对假体外旋角度及其在性别间影响均无差别,股骨上髁轴线作为股骨假体旋转对线的参照轴较为恒定可靠,而Whiteside's线的变异度较大。 Objective To compare the accuracy of two reference axes, i.e. the transepicondylar axis and the Whiteside's line, in determing the rotational alignment of the femoral prosthesis, in order to reduce the incidence of patellofemoral complications in total knee arthroplasty. Methods Eighty knees of healthy Chinese adults, the included angle (PAPA: between the perpendicular line of anteroposterior line; PCA: between STEA and PCL) were measured to compare the difference between male and female. Results PCA: male 3.56°±1.27°(1.42~6.65°), female 3.45°±1.09°(1.10-5.65°); PAPA: male 3.40°±1.60°(-1.01~6.06°), female 3.99°±2.06°(-0.84~7.76°); There was no statistics meaning on the difference between PCA and PAPA, and there was no statistics meaning on the difference between male and female. Conclusions There was no difference between PCA and PAPA, and there was no difference between male and female. Compared with the Whiteside's line, the transepicondylar axis seems to be more reliable to determine the rotational alignment of femoral component in tatal knee arthropasty.
