围手术期预防性抗菌药物应用调查分析

INVESTIGATION AND ANALYSIS OF THE PROPHYLACTIC ANTIBACTERIALS APPLICATION DURING PERIOPERATIVE PERIOD IN OUR HOSPITAL

目的调查分析围手术期预防性应用抗菌药物的现状,评价用药的合理性,提高临床合理用药。方法回顾性调查分析2008年7月～12月符合围手术期要求的病例623例。结果623例围手术期患者抗菌药物使用率为96.95%,单一用药占63.72%,二联和三联分别为32.26%和4.01%。不合理用药中Ⅰ类切口为53.07%,Ⅱ类切口为60.87%。不合理用药主要表现在药物选择起点高;用药时机不合理;术后用药时间过长;用药超量;未按药物的药动学特征分次给药;药物联合用药不合理等。结论应进一步加强管理,规范应用,保证临床用药的安全、有效和经济。

Objective To analyze the current application of prophylactic antibacterials during perioperative period in our hospital, to appraise the rationality of the application and to improve the clinical antibacterials＇ rational use. Methods A retrospective survey in 623cases from July 2008 to December 2008. Results The antibacterials application ratio of the 623 cases during perioperative period is 96.95%, including 63.72% of the monotherapy, 32. 26% of the double - therapy and 4. 01% of the triple - therapy. Irrational application in operation incision type Ⅰ is 53. 07%, and typeⅡ is 60. 87%. Irrational medication lies mainly in：the drug selection starting point is too high; the medication timing is unreasonable; the postoperative medication is too long; medication is excessive; divided doses is not in line with pharmacokinetie characteristics ; drug combination is unreasonable etc. Conclusions The antibacterials medication management should be further strengthened and standardized and the clinical medication should be safer, more effective and more economic.
