摘要
目的探讨全氟丙烷气体联合黏弹剂在复杂性眼外伤手术中的应用。方法回顾性分析42例(42只眼)复杂性眼外伤患者,术中应用黏弹剂回纳脱出的虹膜,术毕前房注入全氟丙烷(c鼎)气体0.2ml。观察术后前房形成情况,虹膜复位情况,前房气泡消失时间,角膜及眼压等。结果术后全部患者前房形成良好。20例(47.62%)瞳孔基本恢复圆形,无虹膜前或后粘连;16例(38.10%)瞳孔变形,轻度虹膜前粘连;6例(14.28%)瞳孔严重变形及部分虹膜缺损。前房内C,F8于术后1~30d消失,平均消失时间为(17.05±7.33)d。术后角膜均水肿,2周后明显减轻。8例术后一过性高眼压,经治疗1周后眼压正常。结论眼外伤清创缝合术中前房应用黏弹剂,术毕注入C3F8气体,两者联合应用,是复杂性眼外伤前房重建的有效方法。
Objective To evaluate the effects of perfluoropane (C3F8) gas combined with viscoelastic agents for complicated eye trauma. Methods Retrospective reviewed 42 cases (42 eyes) with complicated eye trauma. Viscoelastic was used during operation and 0.2ml C3F8 gas was injected intracamerally when the opera- tion was over. Observing the formation of anterior chamber (AC), the recovery of iris, the time when C3F8 gas disappeared, cornea and intraocular pressure. Results All 42 patients recovered the depth of anterior cham- ber. Among them, 20 cases (47.62 % ) maintained round pupil without anterior or posterior adhesion of iris; 16 cases (38.10%) got deformed pupil and slight anterior adhesion of iris; 6 cases ( 14.28% ) got severe pupil deformation and part coloboma of iris. The gas bubble resolved 1-30 days later, average ( 17.05± 7.33 ) days. All cases suffered serious edema of comea in the early time, and relived 2 weeks later. Eight cases showed postop- erative ocular hypertension and were controlled in one week. Conclusions The application of perfluoropane (C3Fs) gas combined with viscoelastic contribute to form anterior chamber, increase the security of the opera- tion, decrease the complications of anterior or posterior adhesion of iris, hyphema and improve the visual function.
