鼻中隔矫正术后鼻中隔黏膜瓣缝合和鼻腔填塞对比研究

Comparison on effectiveness of suture and nasal packing after septoplasty

目的比较鼻中隔矫正术后鼻中隔黏膜瓣连续缝合和鼻腔填塞对患者术后症状、并发症和住院时间的影响。方法25例诊断为鼻中隔偏曲,并排除其他鼻部疾病和基础疾病的成年患者,入院接受鼻中隔矫正术,术后随机接受缝合(13例)或者填塞(12例),比较术后第一天症状(鼻部头部疼痛和睡眠情况自我评分),并发症和住院时间。结果缝合组较填塞组术后头部和鼻部疼痛有明显减轻,睡眠显著优于填塞组(P<0.05)。两组均无观察到鼻中隔血肿、脓肿和穿孔。缝合组术后住院时间平均为1.3d(1～3d),填塞组为2.7d(2～4d),两组间无显著差异(P>0.05)。结论鼻中隔矫正术后黏膜瓣缝合可作为鼻腔填塞外的另一选择,值得临床推广使用。

Objective To compare post -operative symptoms, complications and hospitalization time of patients using suture or nasal packing after septoplasty. Methods Twenty - five adult patients diagnosed as nasal septal deviation and excluded other nasal and general diseases were randomly received septal suture （ n = 13 ） or nasal packing （ n = 12 ） after septoplasty. Symptoms on the first day after surgery, complications and hospitalization time were compared between these two groups. Results Symptoms of headache and nasal pain were more severe in packing group than suture group （ P 〈 0.05 ）. Sleep in patients of packing group was worse than that of suture group （ P 〈 0.05 ）. There were no complications such as septal hematoma, abscess and perforation happened in patients of both two groups. Difference in hospitalization time in these two groups was not significant （ suture group 1.3 days, packing group 2.7 days, P 〉 0.05）. Conclusion Besides nasal packing, septal suture can be another choice after septoplasty.
