腹腔镜与开腹手术治疗宫外孕术后的临床护理分析

Analysis on clinical nursing of laparoscopic operation and laparotomy for ectopic pregnancy

目的比较腹腔镜手术与开腹手术治疗宫外孕患者术后护理的不同,提高护理的有效方法和质量。方法随机选取120例宫外孕术后患者,分为腹腔镜和开腹两组,比较两组患者手术时间、离床活动时间、排气时间、疼痛时间、留置尿管时间、术后体温及住院天数。结果行腹腔镜手术组与行开腹手术组对比,年龄、产次、孕次、术前血β-HCG水平无明显统计学差异(P>0.05)。腹腔镜组手术时间(54±10.6)min,开腹组为(51.0±11.2)min,无统计学意义(P>0.05)。腹腔镜组离床活动时间、排气时间、疼痛时间、留置尿管时间、术后体温及住院天数均少于开腹组(P<0.01)。结论腹腔镜手术组患者的术后恢复快,所需时间短,有效提高了护理水平,但技术要求高、视野范围小、操作范围窄、费用高是其不足。

Objective Postoperative care for patients with eclopic pregnancy after laparoscopic surgery and laparotomy was studied in order to raise the effectiveness and quality of nursing care. Methods One hundred and twenty patients with ectopic pregnancy after operation were randomly divided into two groups： laparoscopy group and laparotomy group, and their active time after leaving bed,operating time, time for detaining urethral catheterization and aerofluxus time,duration for pain and hospitalization days were compared. Results There was no difference found between these two groups in patient age, parity and preoperative serum β - human chorionic gonadotrophin levels （ P 〉0.05）. The duration for pain, period for passing gas,the time of hospitalization and leaving bed after operation and the case of fever were all significantly 1 ess than those of laparotomy group （ P 〈 0. 01 ）. While the time for operation showed no difference （ P 〉 0.05 ）. Conclusion Patients in laparoscopy group can be refreshed in shorter time after postsurgical care than that of patients in laparotomy group, and it is beneficial for raising quality of nursing care. On the other hand,it also has some disadvantages such as complicated operative techniques, limited view scope, small operation space and high cost.
