摘要
目的探讨系统性红斑狼疮(SLE)患者的临床表现特点,并对疾病活动性指标进行分析。方法回顾性分析112例SLE患者的临床资料及相关的实验室检测指标,按不同性别、疾病活动指数分组,分析不同性别的患者临床表现及免疫学检查结果,比较不同病情的患者SLE的活动指标。结果SLE女性患者面部皮疹、脱发、光过敏的发生率显著高于男性,而男性患者系统性损害(如肾脏病变、心脏及消化系统损害)的发生率显著高于女性患者,差异均有统计学意义(P<0.05)。女性患者抗SSA抗体阳性率和抗U1RNP/Sm抗体阳性率显著高于男性患者,差异有统计学意义(P<0.05)。病情不同,SLE患者血清补体C3水平及白细胞计数间差异有统计学意义(P<0.05)。结论男女SLE患者临床表现具有明显差异,男性患者早期特异性表现不典型,但器官损害重;白细胞与补体C3的降低与疾病的活动程度明显相关,可以作为临床上监测病情的重要指标。
Objective To explore the clinical features of patients with systemic lupus erythematosus(SLE)and analyze the activity indexes of this disease.Methods Using the retrospective method to analyze the clinical data and related laboratory indexes of 112 cases with SLE.All cases were divided into different groups based on different gender and activity index of the disease,then comparing the clinical features and the results of immunologic test between different genders and the active laboratory results among patients with different pathogenetic conditions.Results The incidences of facial rash,erythema,alopecia,and photosensitivity were higher in female patients than in male ones,while the damages of systemic organs,such as renal lesions,damages of heart and digestive system,were much more serious in the male patients,which had a statistical significance(P〈0.05).The positive rates of anti-SSA antibody and anti-U1RNP/Sm antibody in the female patients were significantly higher than the male ones(P〈0.05).The level of complement C3 and the count of leukocyte were significantly different according to the diverse severity of SLE(P〈0.05).Conclusion There are remarkable differences of clinical manifestations in the male and female patients with SLE.It′s not typical of the specific performance of SLE in male patients in early stage,but the damages of organs present more serious.The level of complement C3 and the count of leukocyte were related with the severity of SLE,which can be used as indicators for monitoring the disease.
