摘要
目的探讨心率变异性与高血压患者靶器官损害的相关性。方法分别检测高血压伴靶器官损害患者(实验组75例)、高血压患者不伴靶器官损害患者(对照组40例)24h动态心电图(SDNN、SDANN、LF、HF)、心脏彩超(LVM、LVMI)、颈动脉彩超、血生化、24h尿蛋白定量等指标。结果实验组SDNN、SDANN、RMSSD、HF低于对照组,实组组LF高于对照组,两组比较差异有统计学意义(P<0.05)。实验组75例中左室肥厚者35例,对照组75例有10例左室肥厚。实验组颈动脉壁增厚率73.33%,对照组为10.67%。两组相比有统计学意义(P<0.01)。实验组24h尿蛋白定量与对照组相比有统计学意义(P<0.01)。实验组与对照组血糖相比无统计学意义(P>0.05),肌酐、TC、TG、LDL-C两组相比有统计学意义(P<0.05)。结论高血压患者HRV程度与其靶器官功能损害程度有密切关系,定期观察高血压患者HRV与血压变化情况,并积极改善自主神经功能,对防治靶器官损害十分必要。
Objective To explore the heart rate variability in patients with hypertension related target organ damage.Methods Detection of hypertension patients with hypertension related target organ damage(experimental group,75 cases),hypertension patients without hypertension related target organ damage(control group of 40 persons) 24-hour ambulatory electrocardiogram(SDNN,SDANN,LF,HF),echocardiography(LVM,LVMI),carotid artery color Doppler ultrasound,blood biochemistry,24-hour urinary protein were measured.Results The experimental group SDNN,SDANN,RMSSD,HF lower than the control group.The experimental group LF higher than control group.The difference was statistically significant(P0.05).75 cases of the experimental group had 35 cases of left ventricular hypertrophy.75 cases of the control group had 10 cases of left ventricular hypertrophy.The rate of carotid artery wall by 73.33% experimental group,but 10.67% in the control group.Compared the two groups was statistically significant(P〈0.01).24-hour urinary protein of experimental group compared with the control group was statistically significant(P〈0.01).Blood glucose in the two groups was no statistically significant(P0.05).Creatinine,TC,TG,LDL-C compared to the two groups was statistically significant(P0.05).Conclusion HRV in patients with hypertension damaging of their target organ function is closely related.To regular observation of HRV in patients with hypertension and blood pressure changes,and actively improving the autonomic nervous system function,prevention and treatment of target organ damage is necessary.
