眼外伤性视神经挫伤视觉诱发电位的分析

Visual Evoked Potential of Optic Nerve Contusion Following Ocular Trauma

目的探讨视觉诱发电位(visual evoked potential,VEP)在眼外伤性视神经挫伤中的诊治应用价值。方法对80例(80眼)外伤性视神经挫伤患者进行VEP测定,并与自身健侧眼进行对照。结果80例(80眼)VEP检测正常16眼(20.0%),异常64眼(80.0%)(χ2=3.95,P<0.05);VEP异常表现为:VEP波形完全消失10只眼,单纯P100波幅降低40只,P100潜伏期延长30只眼;轻度眼受伤波幅、潜伏期与重度眼受伤比较有显著性差异(t=3.614,t=3.518,P均<0.05);两组伤后检查时间与P100潜伏时间和波幅的改变均无相关性((r=0.039和r=0.015,P均>0.05)。结论VEP检查能准确客观地反映出伤眼的视功能,是临床诊断治疗的重要依据。

Objective To explore the diagnosis and treatment value of the visual evoked potential（VEP）for caused by ocular trauma. Methods The optic nerve contusion following ocular trauma was determined in 80 patients （80 eyes,as observation group）and was compared with the VEP of the contralateral healthy eyes（80 eyes as control group）. Results The VEP determination showed that 16 eyes（20.0%）were normal in 80 patients（80 eyes）and 64 eyes（80.0%） abnormal（X^2=3.95,P〈0.05）. The VEP abnormalities showed that the VEP wave disappeared completely in 10 eyes, simple P100 amplitude was decreased in 40 eyes and P100 latent period extended in 30 eyes. There was a significant difference in the amplitude and latent period between mild eye injury and severe eye injury（t=3.614,t=3.518,P are〈0.05）. The time of examination after injury had no correlation with and the changes in the P100 latency and amplitude between two groups（r=0.039 和 r=0.015, P〉0.05）. Conclusion The VEP determination can accurately reflect the visual function of injury eyes ,which is an important basis for clinical diagnosis and treatment
