健脾益肾活血法治疗维持性血液透析患者营养不良30例临床观察

Therapeutic Effect of Therapy of Strengthening Spleen,Tonifying Kidney and Activating Blood for Malnutrition Induced by Maintenance Hemodialysis:An Observation of 30 Cases

目的:观察健脾益肾活血法对维持血液透析(MHD)营养不良患者营养状况的作用。方法:将58例MHD营养不良患者随机分为2组。治疗组30例,在优质高蛋白饮食及充分透析基础上给予健脾益肾活血颗粒治疗,每天2次。对照组28例,饮食及透析治疗同前,但不予中药治疗。2组均治疗90天,测定2组治疗前后部分营养不良指标。结果:治疗组治疗后白蛋白(Alb)、前白蛋白(Pre-Alb)、转铁蛋白(TRF)、血红蛋白(Hb)值明显升高,与治疗前比较,差异有显著性或非常显著性意义(P<0.05,P<0.01),2组治疗后各项营养指标比较,差异有显著性或非常显著性意义(P<0.05,P<0.01)。治疗组营养不良评分明显降低,营养状态好转,与治疗前比较,差异有显著性意义(P<0.05);2组治疗后营养不良评分比较,差异有显著性意义(P<0.05)。治疗组治疗后消化道症状积分明显改善,与治疗前比较,差异有显著性意义(P<0.05);2组治疗后消化道症状积分比较,差异有显著性意义(P<0.05)。结论:采用健脾益肾法能较好地改善营养不良状态,可作为MHD患者临床治疗营养不良的有效方法。

Objective:To observe the therapeutic effect of therapy of strengthening spleen,tonifying kidney and activating blood for malnutrition induced by maintenance hemodialysis(MHD).Methods:Fifty - eight MHD - induced malnutrition patients were randomized into the treatment group(n =30) and the control group(n =28).The two groups received maintenance hemodialysis combined with high - quality and high protein diet.Additionally,the treatment group(n=30) received herbal medicine granules with the actions of strengthening spleen,tonifying kidney and activating blood,twice a day.The treatment lasted 90 days.Partial parameters of malnutrition were examined before and after treatment.Results:After treatment,pre - Alb,Alb,TRG and Hb were increased in the treatment group(P<0.05 or P<0.01 compared with those before treatment),and the differences were significant between the two groups(P<0.05 or P<0.01).The score of malnutrition and digestive tract symptoms was improved in the treatment group after treatment(P<0.05 compared with that before treatment and that in the control group after treatment).Conclusion:Therapy of strengthening spleen,tonifying kidney and activating blood for malnutrition is effective for malnutrition induced by MHD.
