前哨淋巴结活检替代腋窝淋巴结清扫术在乳腺癌治疗中的应用研究 被引量：13

摘要 目的探讨前哨淋巴结活检术(SLNB)替代腋窝淋巴结清扫术(ALND)在早期乳腺癌患者治疗中的应用价值和安全性。方法应用亚甲蓝进行SLNB,2007年3月到2009年3月始收治的临床T1～2N0M0乳腺癌病人入选本研究。所有患者均签署知情同意书,不同意SLNB替代ALND治疗为A组;同意SLNB替代ALND病人为B组,依据前哨淋巴结(SLN)状况,对SLN阴性者仅行SLNB,SLN阳性者行ALND。结果2007年3月至2009年3月共入组55例病人,29例SLNB替代ALND病人术后上肢水肿、疼痛、麻木、僵硬、肩关节活动受限及肌力减退方面的并发症均明显较ALND减少,在随访的13.7个月中,未见1例腋窝复发。结论前哨淋巴结活检术替代腋窝淋巴结清扫术术后并发症明显减少,复发率低,是早期乳腺癌患者的理想术式。
