脂肪抽吸术对脂源性激素、炎性标志物及胰岛素抵抗的影响

Effect of liposuction on adipokines, inflammatory markers and insulin resistance

摘要 目的探讨脂肪抽吸术对脂源性激素、炎性标志物与胰岛素抵抗的影响,并探讨其临床意义。方法对16例接受腹部脂肪抽吸手术患者术前及术后3个月的血清脂联素、抵抗素、c反应蛋白、白细胞介素-6、肿瘤坏死因子-α及胰岛素敏感性进行检测并比较。结果血清脂联素术后[(5.56±1.03)ng/ml]较术前[(6.54±1.27)ng/ml]明显升高(P〈0.05);抵抗素术后[(6.30±3.25)pg/ml]较术前[(5.18±2.28)pg/ml]明显下降(P〈0.05);胰岛素敏感性术后(-3.78±0.39)较术前(-3.48±0.32)明显增强(P〈0.05);TNF—α术后[(0.19±0.02)pg/ml]较术前[(0.23±0.03)pg/ml]明显升高(P〈0.01);IL-6术后[(0.27±0.04)pg/ml]与术前[(0.31±0.06)pg/ml]比较,差异无统计学意义(P〉0.05);CRP术后[(8.03±6.01)mg/L]与术前[(7.28±6.21)mg/L]比较,差异无统计学意义(P〉0.05)。结论腹部脂肪抽吸术在去除大量皮下脂肪的同时,对脂源性激素及胰岛素抵抗等也有非常有益的影响。 Objective To study the effect of large-volume liposuction on adipokines,inflammatory markers and insulin resistance and its significance. Methods The levels of serum adiponectin, resistin, IL-6, ANF-α, CRP and insulin sensitivity were measured preand 3 months postoperatively in 16 consecutive patients undergoing liposuction. Results The adiponectin postoperative [ (5.56 ± 1.03) rig/ ml] was significantly higher than the preoperative [ (6.54 ± 1.27 ) ng/ml ] ( P 〈 0. 05 ) ; the resistin postoperative[ (6.30 ± 3.25 ) pg/ml ] is significantly lower than the preoperative [ (5. 18 ± 2.28 ) pg/ml ] (P 〈 0. 05) ; the insulin sensitivity postoperative ( - 3.78 ± 0. 39 ) significantly increased compared to the preoperative( -3.48 ±0. 32, P 〈0. 05) ; the TNF-α postoperative [ (0. 19 ± 0. 02) pg/ml] significantly increased compared to the preoperative [ (0. 23 ± 0. 03 ) pg/ml, P 〈 0. 01 ) ] ; the postoperative IL-6 [(0.27 ± 0.04) pg/ml] was similar to the preoperative[(0.31 ± 0.06) pg/ml, P 〉0.05]; the posoperative CRP[ (8.03 ± 6. 01 ) mg/L] was not significantly different with the preoperative [ (7.28 ± 6. 21 ) mg/L,P 〉 0. 05 ]. Conclusions Large-volume liposuction has good effect on insulin resistance and adipokines.
