进展期食管癌的光动力治疗

Photodynamic Therapy for Advanced Esophageal Cancer

目的观察光动力疗法治疗进展期食管癌的近期疗效。方法胃镜检查及病理学检查诊断为进展期食管癌患者1例,静脉注射光敏剂PSD-0075mg/kg后6h,应用波长630nm的半导体激光进行光动力治疗,功率密度150mW/cm2,照光时间30min,能量密度270J/cm2。术后5周复查胃镜并进行病理学检查,评价近期疗效。并记录术中和术后不良反应发生情况。结果术后1周患者梗阻症状完全缓解,术后5周胃镜示原病灶完全消退,病理检查未发现肿瘤细胞。术中及术后无出血、穿孔及瘢痕狭窄等不良反应发生。结论光动力疗法治疗进展期食管癌安全、有效、损伤小,术后恢复快,能显著提高患者的生活质量。

Objective To observe the short-term effect of photodynamic therapy for advanced esophageal cancer.Methods After intravenous injection of PSD-007 at 5mg/kg for 6 hours,the patient with advanced esophageal carcinoma was irradiated with a semiconductor laser using a cylindrical fiber（630nm wavelength,power density of 150mW/cm2,irradiation time of 30 minutes,and a total light dose of 270J/cm2）.Endoscopy and pathological examination were conducted to evaluate the short-term effect at 5 weeks after PDT.ResultsObstructive symptoms of the patient disappeared at one week after PDT.Endoscopic examination revealed the original lesion disappeared completely and pathological examination found no tumor cells at 5 weeks after PDT.No adverse events were seen during and after PDT.ConclusionsPDT is a safe and effective treatment for advanced esophageal cancer,and can significantly improve quality of life of patient.
