摘要
目的:探讨丹红注射液对大鼠移植肾缺血再灌注损伤和细胞间粘附分子1(ICAM-1)表达的影响。方法:建立近交系雄性SD大鼠同基因原位肾移植模型。共设4个实验组,假手术组,移植缺血再灌注组,丹红注射液预防组,丹红注射液治疗组。检测术后24 h各组大鼠的移植肾尿素氮(Blood urea nitrogen,Bun)和肌酐(Serum creatinine,Cr)变化,采用免疫组化法检测ICAM-1表达,HE染色光镜下观察肾组织损伤的病理改变,并进行Paller氏评分。结果:与假手术组比较,移植缺血再灌注组肾小管细胞ICAM-1阳性表达增强(P<0.01),Paller氏评分增加(P<0.01),肾小管上皮细胞肿胀、变性和坏死程度严重,ICAM-1表达量与肾小管评分呈正相关(P<0.01);与移植缺血再灌注组及治疗组比较,丹红预处理组肾小管细胞ICAM-1阳性表达下降(P<0.01),Paller氏评分降低(P<0.01),肾小管上皮细胞肿胀、变性和坏死程度较轻。结论:丹红注射液可能通过下调ICAM-1表达,减轻炎症反应对移植肾的损伤。
Objective :To investigate the effects of Danhong Injection on expression of ICAM-1 in rat kidneys with ischemia reperfusion (I/R)injury after kidney transplantation. Methods:Ten normal SD rats were allocated into the sham operation. And the rats were only anesthetized , and kidneys were exposed for 45 minutes. The allogeneic male SD rats model of kidney transplantation were established.The recipients were divided into 3 groups (n=10,in each)at random: I/Rgroup, Danhong pretreatment group, danhong treatment group. Blood urea nitrogen (BUN) and serum creatinine (CR) levels were determined after 24 hours. The expression of ICAM-1 in kidney was quantitatively detected by immunohistochemistry. Results : Compared with those in the Sham group, the expression of ICAM-1 (P 〈0.01 ) and the Paller's grades of kidney tubule (P〈0.01) in I/R model group and Danhong treatment group were significantly increased . The kidney tubular epithelial cells distributed disorderly.The necrosis and pyknosis of kidney tubular epithelial ceils increased very much with inflammatory cell infiltration .The kidney tubular Paller's grades were positively related with the expression of ICAM-1 (r=0.92, P〈0.05 ). ByDanhongpretreatment,theexpressionofICAM-1 and the Paller' s grades of kindey tubule (P〈0.01)weredecreasedsignificantly(P〈0.01). The kindey tubules epithelial ceils swelled mildly. The inflammatory cell infiltration was obviously relieved. Conclusion: Danhong injection can decrease renal ischemia reperfusion injury after renal transplantation, possiblely by down-regulating ICAM-1 expression.
