膝关节置换术后早期夹闭伤口引流管对引流量的影响

Study for the effect of drainage volume by early close wound drainage tube for patients with postoperative knee replacement

目的比较术后早期夹闭引流管对膝关节置换术后伤口引流量及失血量的影响。方法选取80例因骨关节炎进行单侧全膝置换术的患者,随机分为实验组及对照组各40例。实验组术后引流管处于夹闭状态6h,对照组引流管处于持续开放状态。记录术后24h及48h内的伤口引流量、异体血输入量及术后3d血红蛋白含量并进行统计学比较。结果实验组术后24h及48h伤口平均引流量分别为140.5ml及172.6ml,明显小于对照组的387.1ml及456.21ml,实验组病例总输血量为390ml,明显低于对照组的415ml;术前及术后3d血红蛋白含量差值实验组为3.42g/L,明显低于对照组的3.87g/L。结论在单侧人工全膝置换术后早期夹闭伤口引流管,患者关节出血量较少。

Objective To assess the effect on drainage volume and blood loss by early closing wound drainage tube for patients with knee replacement Method 80 patients with unilateral total knee arthroplasty due to osteoarthritis were divided into experimental group and control group, closed the drainage tube in experimental group for 6 hours after the surgery, and kept the drainage tube open in control group. The amount of wound drainage at 24 h and 48h after the surgery, the amount of blood transfusion and hemoglobin content after 3 days were recorded and analyze. Result The average amount of wound drainage in experimental group at 24 h and 48 h postoperatively were 140.5 ml and 172.6 ml, which is significantly less than that of control group （387.1 ml and 456.21 ml）. the average amount of blood transfusion of the experimental group were 390 ml, which is significantly less than that of control group （415 ml）. The average decrease of hemoglobin after the surgery of the experimental group was 3.42 g/L, which is significantly less than 3.87 g/L of the control group. Conclusion During unilateral total knee arthroplasty, closing the drainage tube early after the surgery can decrease the amount of bleeding.
