矢状窦旁脑膜瘤的显微外科治疗(附20例报道)

目的探讨显微外科手术治疗矢状窦旁脑膜瘤的方法和效果。方法回顾性分析自2000年1月～2009年1月显微手术治疗的20例矢状窦旁脑膜瘤的临床资料。结果SimpsonⅠ级切除12例(60%),Ⅱ级切除4例(20%),Ⅲ级切除4例(20%)。术后随访8～96个月,平均22个月,无手术死亡及复发病例。结论利用显微外科手术技术,有效地控制术中出血,并妥善处理好上矢状窦及避免脑皮质引流静脉的损伤,力争全切肿瘤,可提高矢状窦旁脑膜瘤的手术疗效。

Objective To explore the methods and efficacy of microsurgical removal for parasagittal meningiomas. Methods A retrospective analysis of 20 cases with parasagittal meningiomas treated microsurgically was performed from 2001.1to 2009.1. Results According to Simpson classification, 12 cases were Simpson Ⅰ resection（60%）, 4 cases Simpson Ⅱ（20%）, 4 cases Simpson Ⅲ（20%）. Follow up for a mean 22 months （ranged 8-96 momths）, no death and recurrence were revealed. Conclusion Application of microsurgical techniques, efficient management of operative bleeding, dealing with the superior sagittal sinus and avoiding the damage of cortical veins, removal the tumors completely, are effective strategies to improve the surgical efficency.
