27例严重肝外伤治疗体会

THE EXPERIENCE OF TREAMENT OF SERIOU LIVER TRAUMA OF 27 CASES

目的探讨严重肝外伤病人的手术治疗方式。方法回顾性分析我院1998年至2008年来27例严重肝外伤治疗情况,其中Ⅲ级21例,Ⅳ级4例,Ⅴ级2例。结果治愈25例,治愈率92.59%,死亡2例,死亡率为7.4%。术后并发症9例,占33.33%。结论对肝破裂病人的外科处理关键是:①尽早施行手术探查;②充分暴露肝创面以利控制出血,根据肝损伤程度及病人情况选择不同术式。

Objective To investigate the operation of serious hepatic trauma.Methods 27patients with hepatic trauma in our hospital between 1998~2008 were retrospectively reviewed.21 cases were Ⅲ-grade,4 cases were Ⅳ-grade,2 cases were Ⅴ-grade.Results 25 patients survived（92.59%）.2 patients died,（7.4%）.The morbidity rate is 33.33%（9/27）.Conclusion The most important in the operation treatment of hepatic trauma is：① Execute the surgery as soon as possible.② expose the hepatic wound fully in order to control bleeding,and choose different method according to the degree to the trauma and the state of the patient.
