2006～2008年杭州地区抗感染药用药分析

Utilization of Anti-infectives in Hangzhou Area in 2006～2008

目的:了解目前杭州地区医院抗感染药用药现状,分析用药特点及发展趋势。方法:根据杭州地区(汇总了11家三甲医院和9家二甲医院)2006～2008年的抗感染药用药数据,采用金额分析法和数据汇总法进行统计分析。结果:杭州地区抗感染药总金额3年内呈逐年上升趋势,2008年杭州地区首次达到了9.5亿元,上升幅度分别为24.7%、28.7%。2008年抗感染用药排名前5类的是头孢菌素类、氟喹诺酮类、青霉素类、抗真菌药、碳青霉烯类,共占总金额的比例为77.95%;用药金额增率排序前3位的类别是单环内酰胺类、碳青霉烯类、其他类抗生素,减速最快的分别是酰胺醇类、林可酰胺类、硝基呋喃类;用药排名前5位的分别是头孢米诺钠、头孢替安、左氧氟沙星、依替米星、舒巴坦钠/头孢哌酮,共占总金额的比例为20.50%,且均为抗感染药中的二线及三线用药。杭州地区抗感染药品更新较快,新增和淘汰部分抗感染药品,除此之外,2008年用药金额增率最大的药品为头孢孟多,负增率最大的是奥司他韦。结论:抗感染药用药金额逐年稳步增长;抗感染药使用较集中,以二线抗感染药使用金额最大;抗菌谱广、疗效好、不良反应少以及耐药性好的抗感染药使用是追求目标及必然趋势。

OBJECTIVE：To probe into the status quo,characteristics and tendency of the utilization of anti-infectives in Hangzhou area. METHODS： The utilization data of anti-infectives in 11 Three-A class hospitals and 9 Two-A class hospital in Hangzhou were statistically analyzed by consumption analysis and data pooling method. RESULTS： The consumption sum of anti-infectives in Hangzhou area showed an uptrend over the 3 years,increased by 28.7% to 950 million Yuan in 2008. Cephalosporin,quinolones,penicillins,antifungal agents and carbapenems ranked among the leading 5 anti-infectives which accounted 77.95% of the total in 2008. Leading the first 3 places in terms of growth rate of consumption sum in 2008 were monocyclin β-Lactams,carbapenems and other sort of antibiotics,with chloramphenicols,lincosamide and nitrofurans dominating the lead in terms of decreasing speed of consumption sum. Cefminox,Cefotiam,Levofloxacin,Etimicin and Cefoperazone/ Sulbactam were the leading 5 anti-infectives used in 2008 in Hangzhou area,which occupied 20.05% of the total,all were second line and third line anti-infectives. In Hangzhou area,the anti-infectives renewed frequently,some new kinds were added while some others were out of use,moreover,in 2008,Cefamandole showed the highest growth rate of consumption sum while Oseltamivir showed the highest decreasing rate of consumption sum. CONCLUSION： The consumption sum of anti-infectives in Hangzhou area witnessed a steady growth,with the second-line anti-infectives taking the lead in terms of consumption sum. The anti-infectives with broad antibacterial spectrum,better therapeutic efficacy,fewer adverse effect and good drug tolerance enjoy great potential market.
