The application of sinus heart rate turbulence in coronary heart disease

摘要 目的探讨冠心病患者窦性心率震荡(HRT)现象,旨在评价其对冠心病的预测价值。方法人选冠心病患者60例(观察组),同期体检者排除器质性心脏病60例(对照组),两组年龄和性别差异无统计学意义。全部人选者均接受BETHUNE十八导联24h动态心电图(DCG)记录,进行处理后测量计算窦性心率震荡初始(TO)和震荡斜率(Ts)两项指标。结果冠心病患者(观察组)HRT明显消失或减弱,TO明显升高,TO大于对照组(P〈0.01);观察组Ts明显下降,偈大于对照组(P〈0.01)。对照组TO、TS与性别无相关性(P〉0.05),与年龄有相关性(P〈0.05);观察组TO、TS与年龄、性别均无相关性(P〉O.05)。结论冠心病患者HRT明显消失或减弱。HRT可能成为评价冠心病患者自主神经功能状态,预测其危险程度的新指标。 Objective To explore the predictive value of sinus heart rate turbulence (HRT) in patients with coronary heart disease. Methods 60 patients with coronary heart disease were selected as observation group, and control group was 60 cases with no organic heart disease. The two groups had no statistical difference in age and sex. All patients were interviewed by BETHUNE 18-lead 24-hour ambulatory electrocardiogram (DCG) records, and sinus heart rate turbulence(turbulence onset, TO) and turbulence slope (turbulence slope, TS) were measured. Results HRT apparently disappeared or weakened in coronary heart disease patients (observation group), and TO was significantly higher than control group (P〈0.01). TS decreased significantly in observe group (P〈0.01). In control group, TO and TS and were not correlated with gender (P〉0.05), but correlated with age (P〈0.05). In observation group, TO and 'IS were not correlated with age and gender (P〉0.05). Conclusion HRT apparently disappeared or weakened in patients with coronary heart disease, HRT may be used to evaluate the autonomic nervous functional status in patients with coronary heart disease and may be a new risk predictor.
