摘要
目的探讨色素上皮衍生因子(PEDF)、血管内皮生长因子(VEGF)、结缔组织生长因子(CTGF)在脂肪肝患者的发病机理及病程进展中表达的相关性。方法选择经诊断为酒精性脂肪肝患者119例、非酒精性脂肪肝患者141例;同时选择40例健康正常人作对照组。应用酶联免疫分析法(ELISA)对PEDF,VEGF,CTGF进行检测。结果酒精性脂肪肝患者和非酒精性脂肪肝患者的CTGF表达显著上调(P〈0.01),较正常对照组差异有统计学显著性意义(P〈0.01),且与病情的进展呈正相关;而PEDF与VEGF表达明显下调,较正常对照组差异有统计学显著性意义(P〈0.01),且与病情的进程呈正相关。结论PEDF是有效的血管生成抑制剂,对保持正常组织透明无血管的状态有重要作用,CTGF是促进纤维化的重要细胞因子,调节肌成纤维细胞分化和胶原合成,在肝纤维化发生发展中起着重要的作用。而VEGF是血管和淋巴内皮细胞的生长和分化及正常和异常血管新生的重要的调节因子,在生理和病理性血管新生中起重要作用。三者同时表达异常,可能提示脂肪肝患者免疫病理机理中的细胞活化模式,对脂肪肝患者的免疫机制和治疗有指导意义。
Objective To approach the role of pigment epithelium-derived factor(PEGF) ,vascular endothelial growth factor (VEGF) and connective tissue growth factor(CTGF) in onset of fatty liver and the correlation in the expression of PEDF, VEGF and CTGF in the development of fatty liver. Methods To detect PEDF,VEGF and CTGF by ELISA in 119 patients with alcoholic fatty liver, 141 patients with non-alcoholic fatty liver and 40 health adults. Results The expression of PEDF and CTGF in patients with alcoholic fatty liver and non-alcoholic fatty liver was significantly higher than that of control group (P〈0.01). There was positive correlation between expression of CTGF and degree of fatty liver (P〈0.01). The expression of PEDF and VEGF in patients with alcoholic fatty liver and non-alcoholic fatty liver was significantly lower than that of control group(P〈0.01). There was positive correlation between expression of VEGF and degree of fatty liver (P〈0.01). Conclusion PEDF is effective inhibitor to angiogenesis and play a role in maintaining clearing and avaseularity in normal tissue. CTGF is important cytokine to promote fibrosis,which regulate differentiation of myofibroblast and synthesis of collagen,and play a role in onset and development of hepatic fibrosis. VEGF is important regulatory factor to growth and differentiation of blood vessel and endothelial cell and important to neogenesis of normal and abnormal blood vessel, which play a role in physiological and pathological neogenesis of blood vessel. Abnormal expression of PEDF,VEGF and CTGF at the same time may reveal the corpuscular activation pattern in immunopathogenesis mechanism of fatty liver, and may play an important role in immunotherapy of fatty liver.
