经食管床颈部食管胃吻合术治疗食管上中段癌(附402例报告)

TREATMENT OF CARCINOMA OF UPPER AND MIDDLE ESOPHAGUS BY CERVICAL ESOPHAGOGASTROSTOMY THROUGH ESOPHAGEAL BED (402 Cases Reported)

目的：进一步了解采用经食管床颈部食管胃吻合术治疗食管上中段癌的意义及注意事项。方法：回顾总结近年收治４０２例食管上中段癌临床资料，并结合文献分析。结果：术后并发症发生率５７％，手术死亡率０５％，术后３、５、１０年生存率达７６７％、３４１％和２０８％；１１例７０岁以上老年患者随访１～３年，生活质量未降低，术前术后肺通气功能无明显差异。结论：经食管床颈部食管胃吻合术符合生理要求，径途短，操作简便、安全性高、并发症少，尤适心肺功能差。

OBJECTIVE: To further understanding of treatment of carcinoma of upper and middle esophagus by cervical esophagogastrostomy through esophageal bed.METHOD:The data of 402 patients with upper and middle esophageal carcinoma has been reviewed and analysed.RESULTS:5 7% patients occured complications postoperatively and 0 5% patients died intraoperatively.Survival rate of three,five and ten years was 76 7%,34 1% and 20 8% respectively.11 patients over 70 years old were found normal pulmonary ventilation function during one to three year follow-up.CONCLUSION:The operation is simple,safe and less complicated.It is especially suitable for patient with bad cardiopulmonary function and other chronic diseases as well as upper esophageal carcinoma.
