超声内镜在食管黏膜下肿瘤诊断和治疗中的价值

Value of Endoscopic Ultrasonography in the Diagnosis and Therapy of Submucosal Tumors

目的探讨超声内镜在食管黏膜下肿瘤诊断和治疗中的价值。方法对常规胃镜诊断为食管黏膜下肿瘤78例行超声内镜检查,并根据其起源层次不同,采用内镜或手术治疗。结果该组78例中,超声内镜提示71例为平滑肌瘤,其中18例起源于黏膜肌层,53例起源于固有肌层;囊肿4例,起源于黏膜下层;3例为腔外大血管压迫。18例起源于黏膜肌层的平滑肌瘤均行黏膜切除术,无1例穿孔,53例起源于固有肌层的平滑肌瘤行外科手术治疗。结论超声内镜是检查食管黏膜下肿瘤的最有效方法,在超声内镜指导下进行内镜治疗是黏膜下肿瘤治疗的有效手段。

Objective To evaluate the diagnostic and therapeutic value of endoscopic ultrasonography for submucosal tumors in esophagus. Methods EUS was performed in 78 patients with suspected submucosal tumor and they were treated according to the diagnosis. Results 71 of 78 cases were submucosal tumors and 18 lesions originated from the muscularis mucosa,53 lesions located in the proper muscle layer;4 lesions were esophageal cysts. 3 lesions were great vessels. 18 cases of the muscularis mucosa were given EMR, with no perforation complication ; 53 cases of proper muscle layer were given surgical operation. Conclusion EUS is the best test tool for submucosal tumor and can guide further therapy.
