关节突关节形态与退行性腰椎滑脱的关系

The relationship of lumbar facet joint morphology with degenerative spondylolisthesis

目的:探讨关节突关节形态改变与退行性腰椎滑脱(DS)的关系。方法:选取L4-5退行性腰椎滑脱患者52例和同期无腰腿疼无脊椎滑脱的同年龄段患者56例作为对照,将DS组与对照组的关节突关节面冠状位角做比较;将DS组病例按其关节突关节退变程度分组比较不同退变程度下的关节突关节面冠状位角和滑脱程度的差异;同时做关节突关节面冠状位角与滑脱程度的相关性分析。结果:DS组与对照组的L4-5关节突关节面冠状位角差异有显著性(P<0.01);不同退变程度下的关节突关节面冠状位角差异无显著性(P>0.05),滑脱程度差异有显著性(P<0.01);滑脱程度与关节突关节面冠状位角无相关性(r=0.13,P>0.05)。结论:退行性腰椎滑脱患者的关节突关节的形态变化是腰椎退行性变的结果而非其病因。

Objective： To investigate the relationship of lumbar facet joint morphology with degenera- tive spondylolisthesis （DS）. Methods： 52 patients with degenerative L4-5 spondylolisthesis were studied and 56 patients without low back pain or leg pain and ruled out degenerative spondylolisthesis were select- ed as the control group. The orientation of the L4-5 facet joint between DS group and control group were compared; Correlation between the orientation of the L4-5 facet joint and the degree of spondylolisthesis; and the orientation of the L4-5 facet joint and the degree of spondylolisthesis of different grade of os- teoarthritis were compared. Results： The orientation of the L4-5 facet joint were significantly different in patients with DS group compared with controls group （P〈0.01）; There was no significant difference in the orientation of the IA-5 facet joint of different grade of osteoarthritis （P〉0.05）while the degree of spondy- lolisthesis was different （P〈0.01）. there was no signigicant correlation between the orientation of the IA-5 facet joint and the degree of spondylolisthesis （r=O. 13, P〉0.05）. Conclusion： The changes of lumbar facet joint morphology are results of lumber degeneration but not etiological factor.
