鲍曼不动杆菌耐药性及产β-内酰胺酶分析 被引量:4

摘要 目的调查我院分离鲍曼不动杆菌的耐药性及产β-内酰胺酶情况。方法用K-B法进行药物敏感性检测,表型确证试验检测超广谱β-内酰胺酶(ESBLs),表型筛选法检测高产AmpC酶。结果86株鲍曼不动杆菌对氨苄西林/舒巴坦、头孢哌酮/舒巴坦、亚胺培南和美罗培南耐药率分别为25.6%、9.2%、17.4%和15.1%。对其他10种抗菌药物耐药率(39.5%~67.4%)较高。21株菌(24.4%)检出ESBLs、31株菌(36.0%)检出AmpC酶。结论我院鲍曼不动杆菌耐药率高,应加强药敏检测,指导临床合理应用抗菌药物;其对β-内酰胺类药物耐药与ESBLs、AmpC酶有关。 Objective To investigate the drug resistance and β-lactamases in clinical isolates Acinetobacter baumannii in the Third Hospital of Changsha. Methods The drug sensitivity was performed by K-B method. The ESBLs enzyme and AmpC enzyme were detected by phenotypic confirmatory test and phenotypic screening method,respectively. Results The resistance rates of 86 strains of Acinetobacter baumannii to ampicillin/sulbactam,cefoperazone/sulbactam,imipenem and meropenem were 25.6%,9.2%,17.4% and 15.1%,respectively,but to other 10 kinds of antibiotics were higher (39.5%~67.4%). 21 strains (24.4%) were found to produce ESBLs and 31 strains (36.0%) were found to produce AmpC enzyme. Conclusions The drug resistant rate of Acinetobacter baumannii is high in the Third Hospital of Changsha,so drug susceptibility test should be enforced to guide clinicians applying antibiotics reasonably; its resistance to β-lactam drug is related to ESBLs and AmpC enzymes.
