胰腺外伤并发胰瘘的诊治经验

DIAGNOSIS AND TREATMENT OF THE POSTOTRAMATIC PANCREATIC COMPLICATION OF PANCREATIC FISTILA

为了了解胰腺损伤并发胰瘘的病情发展过程，总结其诊治经验，方法对近10年来的7例胰腺损伤并发胰瘘患者的治疗过程进行总结。结果发病以男性青壮年为主；致伤原因多为上腹部及腰背撞伤、挤压伤；胰瘘自愈时间平均77．4天。结论①胰腺损伤早期诊断有待努力。②胰腺损伤越重、越近胰头、手术越迟胰瘘量越大。③胰瘘量的多少与胰瘘自愈时间无明确关系。胰瘘量越大并不表示自愈的可能性越小或时间越长。④胰瘘期间营养代谢支持和维持水一电解质平衡量治疗成功的关键。

Objective To study the developing course of the pancreatic trauma and fistilas and to know how to treat them.Methods seven Patients with pancreatic trauma and fistilas had been treated in the past ten years and the experiences weresummed up here. Results Most patients are young men; Reasons of trauma were attacks and pressing on lumbo - abdominalregion. It took 77. 4 days in average to get well. Conclusion ①It is necessary for us to make an early diagnosis. ②The more seriously it is hurted or the closer it is near the head of pancreas or the later it is treated,the greater the drainage of pancreaiic fistilasis. ③Much drainage of pancreatic fistilas has nothing to do with the time to get well and doesn't mean less possbilities or longertime it needs to get well. ④The key to treat it is depending on the supporting of nutrition and keeping water electrolyte balance.
