类风湿关节炎患者易感性与HLA-DR基因相关性研究 被引量:5

摘要 目的探讨类风湿关节炎(RA)患者易感性与HLA-DR基因的相关性。方法研究对象为75例RA患者及40例健康体检者,采用PCR-SSP法检测HLA-DR4和HLA-DR53基因分型、BNII全自动特定蛋白分析系统速率散射比浊法和乳胶凝集法同时定量和定性检测类风湿因子,并对结果进行回顾性分析。结果①RA患者HLA-DR4和HLA-DR53等位基因发生的频率数分别为44%和56%,与对照组比较均P<0.01,结果有非常显著性意义。组间比较,P>0.05结果无显著性意义,其DR基因与RA的相对危险率分别为3.70和2.64,HLA-DR4高于HLA-DR53。②75例RA患者其速率比浊法和乳胶凝集法分别有59例阳性和44例阳性,两法经χ2检验,结果均P<0.01,有非常显著性意义;组间比较经χ2检验,P<0.05有显著性意义说明速率比浊法好于乳胶凝集法。③75例类风湿性关节炎患者速率比浊法检测结果为198.59±379.21IU/ml,同对照组比较经t检验,P<0.01结果有非常显著性意义。结论RA患者其HLA-DR基因与RA的发生易感性明显有关,为临床诊断和鉴别诊断提供依据。 Objective To study the correlation of susceptibility to RA and HLA-DR gene. Methods PCR-SSP was used to detect genes of HLA-DR4 and HLA-DR53. RF was quantitatively determined by BN II and qualitatively detected by latex agglutination test. The results were retrospectively analyzed. Results In 75 cases of RA, HLA-DR4 and HLA-DR53 allelic genes frequencies reached 44% and 56%, respectively. Both were significantly higher than those in the controls (P〈0.01). The relative risk of DR genes were 3.70 and 2.64 separately. The positive rate of RF in RA patients with rate nephelometry was much higher than that with latex agglutination test (P〈0.01). The concentration of RF by rate nephelometry was 198.59±379.21IU/ml, and showed much higher compared with that of the controls (P〈0.01). Conclusion HLA-DR gene is significantly related to susceptibility to RA patients, and is helpful for clinical diagnosis.
