可脱性球囊栓塞治疗外伤性颈内动脉海绵窦瘘的临床探讨

Embolization treatment with detachable balloon for traumatic cavernous fistulas

目的探讨可脱性球囊栓塞技术治疗外伤性颈内动脉海绵窦瘘的方法及疗效。方法回顾性分析23例采用可脱性球囊栓塞技术治疗的外伤性颈内动脉海绵窦瘘患者的临床资料。结果23例患者全部治愈。21例一次栓塞成功,2例两次栓塞成功。颈内动脉通畅率为91.3%。随访10月至7年,无复发,无并发症发生。结论可脱性球囊栓塞治疗外伤性颈内动脉海绵窦瘘,操作方便、损伤小、安全可靠,可作为外伤性颈内动脉海绵窦瘘的首选治疗方法。

Objective To explore the endovascular treatment of traumatic carotid cavemous sinus fistula with detachable balloon and to evaluate it＇s therapeutic effect.Methods Retrospectively analyzed 23 patients of TCCF,which were diagnosed by digital subtraction angiography and treated endovascularly with detachable balloon.Results All the cases were cured.In these cases,21cases were embolized one time,two cases were embolized twice.The fistulas of 23 cases were occluded with patency of internal carotid artery（ICA）accounting for 91.3%.Follow-up varied from 10 months to 7 years in all cases of the group.None presented recurrence.No complication was found in all 23 cases.Conclusion Endovascular embolization technique with detachable balloon is the prefered method of treatment for traumatic carotid cavenous sinus fistula.
