摘要
目的归纳分析发热性坏死溃疡性Mucha-Habermann病(FUMHD)的临床、组织病理、免疫组化特点及治疗。方法分析患者的临床资料,并复习相关文献。结果2例患儿皮疹具有多形性,临床有斑丘疹、丘疹、水疱、坏死、溃疡;组织病理示表皮角化不全、基底细胞液化变性、真皮血管炎改变。血清学单纯疱疹病毒(HSV)和/或巨细胞病毒(CMV)IgM抗体阳性。经甲泼尼龙、丙种球蛋白和甲氨喋呤联合治疗后,体温恢复正常,皮疹逐步消退。结论发热性坏死溃疡性Mucha-Habermann病可能与病毒感染相关,对糖皮质激素、甲氨蝶呤联合治疗反应良好。
Objective To describe two cases of Febrile ulceronecrotic Mucha-Habermann＇s disease （FUMHD） , and sum up its features in clinical, histopathologic and immunohistochemical. Method The clinical and histopathological features were analyzed. Result Skin lesions presented with polymorphous lesions, papules, maculopapules, vesicles, ulcers and necrosis. Histopathological findings revealed parakeratosis, vacuolization of the basal cell layer and vasculitis in the dermis. Serologic tests revealed primary infections with herpes simplex virus and/or cytomegalovirus. Both of them were successfully treated with methylprednisolon, gamma immunoglobulin and methotrexate. Conclusion FUMHD may be related to viral infection and have a good response to methylprednisolon plus metbotrexate therapies.
