S-100 b蛋白在小儿病毒性脑炎病情判断和预后评估中的价值

The value of S-100 b protein in children with viral encephalitis in determining state of illness and in evaluating prognosis

目的观察病毒性脑炎急性期和恢复期脑脊液(cerebrospinal fluid,CSF)和血清中S-100 b蛋白含量的变化,探讨S-100 b在病毒性脑炎病情判断和预后评估的临床应用价值。方法选择46例病毒性脑炎患儿为观察组,又分为昏迷组(15例)和无昏迷组(31例),20例无神经系统疾病、需行外科腰麻的患儿作为对照组,采用双抗体夹心ELISA法检测急性期、恢复期CSF和血清中S-100 b蛋白含量,进行比较和动态观察。结果①观察组CSF和血清中S-100 b蛋白明显高于对照组(P<0.01);②昏迷组CSF和血清中S-100 b蛋白明显高于无昏迷组(P<0.01);③观察组急性期CSF和血清中S-100 b蛋白水平明显高于恢复期(P<0.01);④CSF中S-100 b蛋白含量与其血清含量呈正相关(r=0.746,P=0.05)。结论CSF和血清中S-100 b蛋白的含量与病毒性脑炎脑损伤程度正相关,对病毒性脑炎的病情判断和预后评估具有重要的临床价值。

Objective To investigate cerebrospinal fluid and serum S-100 b protein levels in children with viral encephalitis to discuss their values in determining state of illness and in evaluating prognosis.Methods 46 cases with viral encephalitis were collected as observe group and 20 cases patients with operation were collected as control group.46 children with viral encephalitis were divided into two groups according to the severity of disease： coma group（15 cases） and non-coma group（31 cases）.The double-antibodies sandwich ELISA was used to detect S-100 b protein concentrations in CSF and serum in these 46 patients and 20 controls.Results ①CSF and serum S-100b level of observed group were higher than control group（P0.01）.② CSF and serum S-100 b level of coma group were higher than those in non-coma group（P0.01）.③patients with acute viral encephalitis has a significantly higher concentration of S-100 b protein of CSF and serum than recovery（P0.01）.④CSF S-100 b correlated with their serum concentrations（r=0.746,P=0.05）.Conclusion The concentration of S-100b protein of CSF and serum in patients with acute viral encephalitis is relative with brain-tissue damage of viral encephalitis.There are important clinical values in determining state of illness and in evaluating prognosis.
