彩色多普勒在睾丸扭转诊断与复位后存活预测的应用价值 被引量:4

摘要 目的:探讨彩色多普勒血流显像技术在睾丸扭转的诊断与复位后存活的预测价值。方法:26例睾丸扭转患者,术前均经彩色多普勒血流显像诊断,并根据其血流供应情况选择手术方式。结果:26例患者诊断符合率100%,其中发病至确诊〈6 h者16例,7例手法复位,9例手术复位;12~24 h者6例,4例手术复位(其中1例术后在1年内利用彩色多普勒血流显像技术跟踪随访发现睾丸萎缩),2例行睾丸切除术;〉48 h的4例均切除睾丸。彩色多普勒血流显像显示有血流供应的扭转睾丸较健侧血流峰值速度降低,阻力指数升高。结论:彩色多普勒血流显像技术是目前诊断睾丸扭转最快捷、方便、简单、安全有效的方法,且可以在术前对睾丸的损伤程度进行评价,可为手术方式的选择提供重要的信息,同时对扭转睾丸复位后存活的预测也有重要价值。 Objective To study the value of color Doppler flow imaging in the diagnosis of testicular torsion and survival prediction after reset.Method For 26 cases of testicular torsion were preoperatively diagnosed by color Doppler flow imaging,and in accordance with its blood supply to choose surgery style.Results The diagnose accordance rate of 26 patients was 100%.The time from onset to final diagnosis:6 h in 16 cases,including 7 of manual reduction and 9 of surgical reduction;12~24 h in 6 patients,with 4 of surgical reduction(1 patient found testicular atrophy with color Doppler flow imaging in the follow-up study within 1 year),and 2 with routine orchiectomy;48 h in 4 cases,they were all removed testicle.Color Doppler flow imaging showed that the blood flow peak velocity of torsional testicle was lower and resistent index was higher than the uninjured side.Conclusion Color Doppler flow imaging is the most efficient,convenient,simple,safe and effective method in the diagnosis of testicular torsion,furthermore it can evaluate the damage extent of testicle in the preoperation,provide important information for the choice of surgical method,also has important clinical value in predicting survival after reset.
