丙泊酚靶控输注对颅内肿瘤患者围手术期氧化应激和血浆细胞因子的影响 被引量:7

摘要 目的观察丙泊酚靶控输注对颅内肿瘤患者围手术期血浆肿瘤坏死因子α(TNF-α)、可溶性细胞间黏附分子-1(sICAM-1)、一氧化氮(NO)和丙二醛(MDA)等水平的影响,探讨丙泊酚的脑保护作用。方法择期全麻下行颅内肿瘤手术患者60例,年龄30~55岁,体重45~73kg,ASAⅠ或Ⅱ级,术中麻醉维持随机分成两组:丙泊酚组和异氟醚组各30例,丙泊酚组采用血浆靶控输注丙泊酚3~6μg/mL,异氟醚组使用1%~2%的异氟醚持续吸入,两组术中均按需静脉给予芬太尼和维库溴铵,并于手术结束前5min分别停止丙泊酚输注和异氟醚吸入。两组均于麻醉诱导前(T0)、气管插管后1h(T1)、2h(T2)、术后4h(T3)、6h(T4)、24h(T5)各时点抽取静脉血,测定血浆TNF-α、sICAM-1、NO和MDA的含量。结果丙泊酚组血浆TNF-α水平在T0~T3时点呈上升趋势,在T4和T5时点下降;血浆sICAM-1水平在T0~T4时点呈上升趋势,在T5时点下降。异氟醚组血浆TNF-α水平在T0~T4时点呈上升趋势,在T5时点下降;血浆sICAM-1水平在T0~T5时点呈上升趋势。与异氟醚组比较,丙泊酚组血浆TNF-α和sICAM-1水平,在T0时点差异无统计学意义,在T3~T5时点均较低(P<0.05)。丙泊酚组血浆NO和MDA水平在T0~T2时点呈上升趋势,在T3~T5时点下降;异氟醚组血浆NO和MDA水平在T0~T4时点呈上升趋势,在T5时点有所下降。与异氟醚组比较,丙泊酚组血浆NO和MDA水平,在T0时点差异无统计学意义,在T3~T5时点均较低(P<0.05)。结论丙泊酚靶控输注比异氟醚吸入可较好地降低颅内肿瘤患者围术期体内氧化应激和炎症水平,发挥脑保护作用。
