摘要
目的探讨能否根据外周血内皮祖细胞(endothelial progenitor cells,EPCs)CD34^+水平评价高血压病患者动脉血管病变程度。方法高血压病患者组62例,对照组20例。高血压病患者采用Framingham心血管危险因素积分分层心血管危险因素,分为低危组18例,中危组14例,高危组17例,极高危组13例。作外周血循环EPCsCD34^+水平、内皮依赖性血管舒张功能(flow—mediated dialation,FMD)测定并作EPCsCD34^+水平与Framingham心血管危险因素积分、FMD的相关性分析。结果高血压病患者外周循环EPCsCD34^+水平随着其心血管危险程度的增加,逐步下降,各组间比较川差异均有显著性(P〈0.05).高血压病患者肱动脉FMD随着其心血管危险程度的增加显著下降(P〈0.01)。各研究组高血压病患者循环EPCs CD34^+水平与Framingham心血管危险因素积分呈负相关(r=-0.875,P〈0.01),与肱动脉FMD呈正相关r=0.859,P〈0.01).结论高血压病患者循环EPCs CD34^+水平下降与心血管危险因素、动脉血管功能损伤有显著的相关性。说明循环EPCs CD34^+水平可以作为评价高血压病患者动脉血管病变的指标.
Objective To evaluate whether the variations in the number of circulating EPCs CD34^+could be presented as the marker of artery vascular lesion in patients with hypertension. Methods 62 patients with hypertension and 20 healthy people were collected. Hypertension group were stratified by cardiovascular risk factors according to Framingham risk factors Score, and were divided into low-risk group of 18 cases, mid-risk group of 14 cases, high-risk group of 17 cases and much-high-risk group of 13 cases. The level of peripheral circulating EPCs CD34^+ , brachial artery FMD were respectively measured. The relationships between the level of circulating EPCs CD34^+ and Framingham cardiovascular risk factors, brachial artery FMD were analyzed. Results The level of peripheral circulating EPCs CD34^+was significantly lower in hypertension groups than that in control group (P〈0.05). Brachial artery FMD was significantly lower in each hypertension groups than that was in control group (P〈0.01). The scores of FMD and CD34^+ cells were inversely correlated with cardiovascular risks .There was a negative correlation between the level of circulating EPCs CD34^+ with Framingham cardiovascular risk faetors(r = -0.875, P〈0.01), a positive correlation with brachial artery FMD (r = 0.859, P〈0.01). Conclusion The level of circulating EPCs is a predictor of artery vascular lesion in patients with hypertension.
