蛛网膜下腔出血腰池置管脑脊液连续引流术的临床研究 被引量:2

摘要 目的探讨腰池置管脑脊液连续引流术治疗蛛网膜下腔出血(SAH)的临床疗效。方法133例蛛网膜下腔出血患者随机分为观察组(65例)和对照组(68例)。对照组采用单纯脑脊液置换术治疗,治疗组采用脑腰池置管脑脊液连续引流术治疗。观察两组术后15d内头痛缓解时间、CSF压力恢复正常时间、意识清醒时间及1月内脑积水及脑血管痉挛的发生率,评定疗效。结果观察组死亡率为1.5%,低于对照组(16.2%),两组相比差异有显著性(P〈0.05);观察组近期疗效总有效率89.2%,高于对照组(70.6%),两组相比差异有显著性(P〈0.05);观察组术后意识清醒时间、头痛缓解时间、血肿清除时间及CSF压力恢复时间低于对照组(P〈0.05);观察组脑积水和脑血管痉挛的发生率低于对照组(P〈0.05)。结论腰池置管脑脊液连续引流术方法简单,近期疗效好,安全性高,是治疗SAH的有效方法。
