预注高渗氯化钠羟乙基淀粉40注射液对法洛四联症麻醉诱导的影响

摘要 目的观察预注高渗氯化钠羟乙基淀粉40注射液(霍姆)对法洛四联症(简称法四)麻醉诱导期缺氧发作的预防效果。方法选择全麻下行法四根治术的患者40例,随机分为两组,在麻醉诱导前分别输注高渗氯化钠羟乙基淀粉40注射液5ml/kg(观察组),乳酸钠林格氏液10ml/kg(对照组)。监测进入手术窒后及麻醉诱导后5、10、15min的平均动脉压(MAP)、血氧饱和度(SaO2)、动脉血氧分压(PO2)、剩余碱(BE),观察麻醉诱导前后的电解质、血红蛋白(Hb)、红细胞比容(Hct),并记录麻醉诱导期使用苯肾上腺素的剂量。结果观察组在全麻诱导后平均动脉压、血氧饱和度、动脉血氧分压下降、酸中毒情况明显低于对照组(P<0.05),两组患者的电解质(Na+、Cl-),Hb和Hct在诱导后无显著改变。麻醉诱导期使用苯肾上腺素的剂量观察组明显少于对照组。结论采用预注高渗氯化钠羟乙基淀粉40注射液(霍姆)能有效地维持法四全麻诱导期血流动力学的稳定,减少右向左分流,预防缺氧发作,提高诱导期的安全性。
