口腔与胃内幽门螺杆菌基因型关系的研究

The study of the relation of helicobacter pylori genotype between stomach and oral cavity

目的明确牙周基础治疗与口腔幽门螺杆菌（helicobacterpylori,Hp）存在的关系。分析胃Hp与口腔Hp的基因型,以明确胃与口腔的Hp是否具有同源性。方法选择胃Hp阳性并伴菌斑Hp阳性患者（6人）的胃粘膜、菌斑样本共10份（胃粘膜4份,菌斑6份）。对比分析其中5位患者和1位家属胃和口腔PCR产物的序列。结果胃和口腔Hp的序列相比较,相同者1例,可能相同者1例,有1-4个碱基不同者2例。1例患者及其家属口腔中均检出Hp,两者的序列有3个不同。结论口腔中有可能同时存在多个Hp菌株,口腔Hp与胃粘膜Hp基因型相同或基本相同,口腔Hp与胃Hp感染的复发或再感染有关。

Objective： To investigate the relationship between the initial periodontal therapy and the presence of oral helicobacter pylori（Hp） and to definitude the homology of HP in oral cavity and in stomach by comparing the genotype.Methods： Samples of gastric mucosa（four samples） and bacterial plaque（six samples0 were selected from six cases with gastric Hp positive and dental plaque positive.DNA sequence of PCR products of 5 patients and 1 Hp infected patient＇s relative was compared and analyzed.Results： An identical sequence of oral and stomach Hp was found in one patient,most likely identical in 1 case,difference of 1～4 bases in 2 cases.HP was detected from one patient and his relative but they showed three different types of DNA sequences.Conclusion： There may be multi-Hp strain existing in oral cavity.Oral colonization of Hp may imply that there is a risk of the relapses of gastric and duodenal Hp infection.
