新型泪道冲洗器冲洗探通治疗婴幼儿先天性泪道阻塞的临床研究

A clinical study of the treatment of infant congenital lacrimal duct obstruction with new lacrimal duct irrigator

目的研制一种新型泪道冲洗器治疗婴幼儿先天性泪道阻塞,与传统泪道冲洗器进行对比分析,探讨其疗效、并发症及治疗时机。方法选用一次性静脉输液针并对针头适当加工改造,作为婴幼儿泪道加压冲洗之专用器械即新型泪道冲洗器。应用新型泪道冲洗器对1000例(1056眼)先天性鼻泪管阻塞患儿行泪道冲洗,一次冲洗不通即行泪道探通后加压冲洗作为改良组。应用传统泪道冲洗针治疗300例(320眼)此类患儿作为对照组。结果综合改良方法治疗婴幼儿先天性泪道阻塞1056眼,一次性治愈率98.1%,与传统方法320眼一次性治愈率79%比较,统计学差异极显著(P<0.01);并发症明显减少;冲洗、探通的治疗时机不受年龄限制。结论新型泪道冲洗器治疗婴幼儿先天性泪道阻塞具有取位合理、操作简便、安全性好,治愈率高、并发症少、缩短病程的优势,具有临床推广价值。

Objective： To evaluate the curative effect,complication and the treatment opportunity of a new-designed lacrimal duct irrigator on infant congenital lacrimal duct obstruction compared with the traditional lacrimal duct irrigator.Methods： Disposable intravenous infusion needle was reconstructed and functioned as a new type of lacrimal duct irrigator.Irrigation of lacrimal passage with the new lacrimal duct irrigator was performed on 1000 infants（1056 eyes） with congenital lacrimal duct obstruction.In the cases of difficulty to irrigate,the second lacrimal passage irrigation with higher pressure was performed following probing lacrimal passage.300 infant patients（320 eyes） treated with traditional lacrimal syringe needle were served as the control ones.Results： The one time cure rate of congenital lacrimal duct obstruction by using new lacrimal duct irrigator is 98.1%,significantly higher than that（79%） by using the traditional lacrimal syringe needle.Compared with traditional lacrimal syringe needle,treatment with new lacrimal duct irrigator obviously reduced the complication and was not restricted by age.Conclusion： Compared with traditional lacrimal syringe needle,treatment of infant congenital lacrimal duct obstruction with new lacrimal duct irrigator is characterized by a high degree of convenience,security,efficiency,rapidity along with rare complication.
