超声在梗阻性与非梗阻性无精子症鉴别诊断中的作用 被引量:7

摘要 目的以睾丸穿刺活检为标准评价经直肠联合阴囊超声在梗阻性无精子症与非梗阻性无精子症鉴别诊断中的应用价值。方法132例无精子症患者以睾丸穿刺活检结果为根据分为梗阻性无精子症及非梗阻性无精子症两组,均进行阴囊超声及经直肠彩色多普勒超声检查,分析两组患者超声检出病变能力的差异。结果132例无精子症中94例诊断为梗阻性无精子症。两组患者中,梗阻性无精子症患者更易被超声检出远端精道及附睾病变,其差异具有统计学意义(P<0.01);在两组患者中检出睾丸异常的差异无统计学意义(P>0.05)。梗阻性无精子症患者睾丸体积大于非梗阻性无精子症患者(P<0.05)。超声在梗阻性与非梗阻性无精子症鉴别诊断中的灵敏度、特异度和粗符合率分别为,远端精道病变:92.6%、65.8%、84.8%;附睾病变:83.0%、71.1%、79.5%。结论经直肠超声联合阴囊超声评价远端及近端精道、测量睾丸体积,在梗阻性无精子症与非梗阻性无精子症的鉴别诊断中有重要作用。
