体表心电图判断心外膜和心内膜起源的室性心动过速 被引量:4

Utility of the 12-lead electrocardiogram in localizing ventricular tachycardia from epicardial or endocardial origin

摘要 近年来,经射频导管消融室性心动过速(室速)已是多数特发性室速的首选治疗手段,而器质性室速的消融也有了很大进展。然而,随着经导管消融室速研究的深入,发现心外膜室速的发生率并不低,和经心内膜途径消融室速相比,心外膜室速消融的成功率较低,并发症发生率增高。如能在消融术前从室速发作时的体表心电图初步判断出心外膜室速可能,将有助于选择合适的经导管消融适应证患者,选择合适的电生理标测设备、标测和消融导管和消融途径(经心内膜、经心脏静脉系统、经心包穿刺等),从而有助于提高消融成功率,降低复发率和并发症发生率。
