从重型肝炎患者痰中检出脑膜脓毒金黄杆菌blaTEM-116、ant(3″)-Ⅰ耐药基因

Detection of blaTEM-116,ant(3″)-Ⅰof Resistant-related Genes in a Chryseobacterium meningosepticum Strain in Sputa from a Severe Chronic Hepatitis Patient

摘要 目的研究重型肝炎患者痰中脑膜脓毒金黄杆菌(CME)耐药机制。方法应用法国生物梅里埃公司的API鉴定条/PSE5.0药敏条和美国BD公司的Phoenix NMIC/ID-109鉴定/药敏板鉴定和细菌药敏试验,应用PCR法检测分离于重型肝炎患者合格痰标本1株多药耐药CME临床分离株16S rRNA、29种β-内酰胺酶相关基因(bla)、6种氨基糖苷类修饰酶基因(AMEs)、消毒剂/磺胺耐药基因(qacE△1-sul1)、3种整合子基因(intⅠ1、2、3)等39种耐药基因,经测序和同源性分析证实并分析其分布情况。结果该株菌经16S rRNA测序和同源性分析(GenBank注册号为FJ839441)证实为CME;药敏试验,环丙沙星、左氧氟沙星和复方新诺明均敏感,亚胺培南、美罗培南、头孢哌酮/舒巴坦、氨曲南、头孢他啶、头孢吡肟、头孢噻肟、头孢西丁、哌拉西林、呋喃唑酮、阿米卡星和庆大霉素均耐药;经测序和同源性分析证实3种耐药基因阳性[2种bla基因(blaTEM-116、blaGOB-16)、1种AMEs基因(ant(3″)-Ⅰ)],3种耐药基因GenBank注册号分别为FJ839444、FJ839442、FJ839443;其他36种基因均为阴性。结论该株耐药机制为多重机制,主要与3种耐药基因[blaTEM-116、blaGOB-16、ant(3″)-Ⅰ]有关,经检索中文生物医学期刊文献数据库(CMCC)、GenBank和Medline数据库,该项研究是第1篇从CME检出blaTEM-116、ant(3″)-Ⅰ耐药基因的报道,也是第1篇从CME同时检出blaTEM-116、blaGOB-16耐药基因的报道。 OBJECTIVE To study the resistant mechanisms in a Chryseobacterium meningosepticum(CME)(former name Elizabethkingia meningoseptica) strain in the sputa from a severe hepatitis B patient.METHODS The susceptibility of antimicrobial agents were detected.16S rRNA,39 resistant-related genes including 29 β-lactamases genes,6 aminoglycoside-modifying enzymes(AMEs)genes,chlorhexidine resistant gene(qacE△1-sul1)and intergron(intⅠ1,2 and 3)of the CME strains isolated from a severe hepatitis B patient was detected by PCR,and verified by DNA sequencing.RESULTS The strain was CME by 16S rRNA-PCR-DNA sequencing(FJ839441).it was susceptible to ciprofloxacin,levofloxacin and trimethoprim/sulfamethoxazole,but resistant to piperacillin,aztreonam,ceftazidime,cefepime,cefotaxime,cefoxitin,meropenem,imipenem,nitrofurantoin,gentamicin and amikacin.There were positive of 3 kinds of resistant-related genes(blaTEM-116,blaGOB-16 and ant(3″)-Ⅰ).Other 36 kinds of resistant-related genes were negative.CONCLUSIONS The CME is multi-resistant in mechanisms,mainly related to 3 kinds of resistant-related genes blaTEM-116,blaGOB-16 and ant(3″)-Ⅰ.
