934例肿瘤住院患者抗菌药物使用率横断面调查及分析

Antimicrobials Usage in 934 Inpatients:A Transverse Section Survey

目的了解肿瘤专科医院抗菌药物使用情况。方法采用横断面调查方法,对2009年7月22日0∶00～24∶00所有住院肿瘤患者进行抗菌药物使用调查。结果934例住院患者中,抗菌药物使用率为23.02%,联合用药比例为5.46%;在所有使用抗菌药物的患者中,76.28%为单用一种抗菌药物,21.40%为二联使用,2.32%为三联使用;在全院各科室中,以妇外科使用率最高,为77.08%,其次为普外科,为48.75%;外科抗菌药物使用率(53.47%)明显高于医院的整体使用率(23.02%);全院治疗性用药病原学送检率为71.01%。结论医院抗菌药物横断面使用率远低于全国平均使用率,治疗性用药病原学送检率较高,但预防用药使用的抗菌药物级别偏高,特别是外科抗菌药物的合理应用需要进一步加强。

OBJECTIVE To find out the antimicrobial usage in our cancer hospital.METHODS A transverse section survey was made from midnight on July 22,2009 to midnight on July 23,2009 within all of the inpatients.RESULTS Among 934 inpatients,the antimicrobials usage was 23.02%,the combined medication ratio was 5.46%.Of all antimicrobial takers,76.28% were given single drug,21.40% double,and 2.32% were given triple.Among all departments of the hospital,the department of surgical gynecology had the highest use rate which was 77.08%,next the department of general surgery（48.75%）.The antimicrobial use rate in surgical departments（53.47%） was remarkably higher than the total hospital use rate（23.02%）.Rate of etiological examination for therapeutic medication in the whole hospital was 71.01%.CONCLUSIONS The transverse section use rate of antimicrobial in our hospital is obviously lower than the national average use rate.The prevention usage of prophylactic medication is on the higher side and the rate of etiological examination for therapeutic medication is on the lower side.Extraordinarily,the management in surgical departments should be further strengthened for rational use of antimicrobials.
