胃癌术后发生血小板减少的原因分析

To analyze the cause of thrombocytopenia after operation of gastric carcinoma

目的分析胃癌术后发生血小板减少的原因。方法回顾性分析2006～2008年间胃癌术后发生血小板减少的病例,观察血小板减少与肿瘤大小、病理分化、淋巴结转移、浆膜浸润、远处转移、手术方式及是否伴有幽门梗阻、有无输血、术前新辅助化疗、腹腔灌注化疗之间的关系。结果血小板减少与肿瘤大小、病理分化、术前新辅助化疗及腹腔灌注化疗无相关性(P>0.05),差异无显著性;与肿瘤的淋巴结转移、浆膜浸润、远处转移、手术方式、有无输血及是否伴有幽门梗阻有相关性(P<0.05),差异具有显著性。结论患者机体的营养状态差、大量输血及骨转移可能是引起血小板减少的主要原因。

Objective To analyze the cause of thrombocytopenia after operation of gastric carcinoma.Methods To observe the relationship between thrombocytopenia and the size of tumer,pathological differentiation,lymph node metastasis,distant metastasis,serosa invasion,modus operandi,pyloric obstruction,neoadjuvant chemotherapy,blood transfusion,intraperitoneal lavage chemotherapy in patients with thrombocytopenia after operation of gastric carcinoma from 2006 to 2008.Results There was no correlation between thrombocytopenia and the size of tumer,pathological differentiation,neoadjuvant chemotherapy,intraperitoneal lavage chemotherapy（P〉0.05）.There was correlation between thrombocytopenia and lymph node metastasis,distant metastasis,serosa invasion,modus operandi,pyloric obstruction,blood transfusion（P〈0.05）.Conclusion the patients with low nutritional status,massive transfusion,osseous metastasis is easily lead to thrombocytopenia
