脐血维生素D水平测定及相关因素分析 被引量:15

摘要 【目的】了解南京地区脐血维生素D水平,探讨影响脐血维生素D的因素。【方法】选取2008年12月—2009年2月在南京出生的足月新生儿102例,记录其出生时的基本情况,采用酶联免疫法测定脐血及部分孕母静脉血25-(OH)D水平,调查孕母在孕后期的维生素D和钙的补充情况。【结果】脐血25-(OH)D水平为(44.34±17.10)nmol/L,以25-(OH)D<30 nmol/L为维生素D缺乏的界值,16例小于该值,占15.7%;30例孕母静脉血25-(OH)D水平为(39.25±15.14)nmol/L,对应的新生儿脐血25-(OH)D为(37.76±12.15)nmol/L,两者呈显著正相关(r=0.842,P<0.001)。孕后期补充维生素D和含维生素D孕妇奶粉大于3个月组,脐血25-(OH)D水平显著高于未补充和补充小于3个月组(P<0.05)。【结论】南京地区脐血维生素D水平高于发展中国家但低于发达国家;孕母维生素D营养状况决定脐血维生素D水平;孕期补充维生素D和含维生素D的孕妇奶粉有助于提高脐血25-(OH)D水平。 [Objective] To understand the status of vitamin D in cord blood and discuss the influence factors. [Methods] 102 newborns born during December 2008 to February 2009 in Nanjing were selected. Their base information and the supplement of vitamin D and calcium in the third trimester were noted. The 25-(OH)D in cord blood and mothers were measured partly by Euzymelinked immunosorbent assay. [ Results] Cord blood 25-(OH)D was (44.34 ± 17. 10) nmol/L. 25-(OH)D〈30 nmol/L were regarded as Vitamin D deficient. 16 newborns (15.7%) had their cord blood 25- (OH)D value under the cutoff. In the 30 mother-infant pairs, the mother serum 25-(OH)D was (39.25!15.14)nmol/L and the cord blood 25-(OH)D was(37.76 ± 12.15) nmol/L, and they had significant positive correlation (r = 0. 842, P〈 0. 001). Gravidas who had adequate intake of vitamin D and formula milk(〉3 months) had newborns with higher cord blood 25-(OH)D(P〉0.05). [Conclusions] The cord blood 25-(OH)D in Nanjing is significant higher than that in developing country but lower than developed country. 25-(OH)D status of puerperant determined the cord blood 25-(OH)D. The supplement of vitamin D and formula milk for pregnant women can improve the level of cord blood 25-(OH)D.
