原发性腹膜后肿瘤术后复发再手术的治疗体会

Reoperation for recurrent retroperitoneal tumor

目的提高复发性腹膜后肿瘤的手术切除率和生存率。方法回顾性分析我院1984年至2004年原发性腹膜后肿瘤术后复发而再手术治疗的28例患者资料,其中良性肿瘤3例,恶性肿瘤25例。共行再手术44例次,完整切除肿瘤35次,姑息切除6次,探查活检3次。结果复发性腹膜后肿瘤患者肿瘤完全切除者的1、2年生存率分别为71.2%和65.3%,而姑息切除和探查活检者均在1年内死亡。结论术后定期随诊、充分的术前准备和受累器官的正确估计是确保手术安全和提高生存率的关键,对复发性腹膜后肿瘤应积极争取完整手术切除。

Objective To improve the resection rate and survival rate for recurrent retroperitoneal tumor （RRT）. Methods The clinical data of 28 cases with RPRT from 1984 to 2004 were analyzed retrospectively. Three cases were benign tumor, and the others were malignant tumor. We performed 44 operations including complete resection in 35 cases, palliative resection in 6 cases and biopsy in 3 cases. Results The 1 - and 2 - year survival rates for completere section eases were 73.7% and 52.6%. The patients who received palliative resection and biopsy died in one year. Conclusion Periodic examination, perfect preparation before operation, and correct evaluation to the involved organs are important to improve operative safety and survival rate. We emphasize the importance of reoperation and complete resection for RRT.
