成人臀肌挛缩症微创治疗临床分析

Clinical Analyze of Minimal Invasive Treatment for Adult Gluteus Contracture

目的探讨成人臀肌挛缩症(AGMC)治疗的必要性及治疗方法。方法回顾性分析128例成人臀肌挛缩症的并发症、疗效等临床资料。结果①成人臀肌挛缩症患者术前均有膝关节磨损、大转子滑囊炎和步态受限等并发症,术后大转子滑囊炎和步态受限消失、膝关节磨损好转。②128例成人臀肌挛缩症患者均用剥离刀微创治疗;A1、A2、B1、B2型疗效1周评分平均为10分;B3、C1型疗效1周评分平均为9分,3周评分为10分;C2型疗效1周评分平均为8分,3周评分平均为9分,6周评分为10分;不同年龄段疗效无明显差异。结论①及时治疗成人臀肌挛缩症是完全必要的。②成人臀肌挛缩症的疗效与年龄无关,只与病情程度、手术的彻底性和康复训练有关。

Objective To explore the necessity and treatment method of adult gluteus contracture（AGMC）.Methods We retrospective analyzed the clinical data（complications,curative effect and so on） of 128 cases of adult gluteus contracture.Results ①All patient suffered from complications in preoperative,such as articular genu abrasions,greater trochanter bursitis and limited run.After the operation,greater trochanter bursitis and limited run about the posture were removed,articular genu abrasions was improved.②The 128 Cases of adult gluteus contracture were treated with minimally invasive using dissection cutter;the curative effect scores of A1,A2,B1,B2 type in one week were 10;the scores of B3,C1 type in one week and three week were 9 and 10,respectively;the score of C2 type in one week,three week and six week were 8,9 and 10,respectively;And there were no difference in different ages.Conclusion It is necessary to treat the adult gluteus contracture in time.The pathogenetic condition,operation and rehabilitation training are concerned in the curative effect of adult gluteus contracture.
