血清子宫内膜抗体及CA_(125)对子宫内膜异位症诊治的研究

An Investigation to the Effect of Endometrial Antilaodies and CA_(125) in Serum on Diagnosis and Therapy of Endometriosis

目的:探讨血清子宫内膜抗体（EMAb）,CA125对子宫内膜异位症（EMT）诊治的临床价值。方法:前瞻性地检测经手术病理证实为子宫内膜异位症的20例两个指标（观察组）,排除EMT的74例EMAb（对照组Ⅰ）及22例CA125（对照组Ⅱ）,结果进行比较,并在观察组中分析术前,术后一周,手术一周服药一月后血清中EMAb及CA125的变化。结果:（1）观察组血清渭度（G±SG）16.6±2.3明显高于对照组Ⅰ1.1±1.0,有非常显著差异P＜0.001;观察组血清CA125（X±s）149.8±60.4明显高于对照组Ⅱ29.6±7.1,有非常显著差异P＜0.01EMAb及CA125用于诊断的敏感性均为100％,特异性分别为91.9％和81.8％。（2）观察组在手术前,手术后一周;手术一周服丹那唑一月后其EMAb分别为（G±SG）16.6±2.3、7.9±2.3、5.5±2.2,CA125各为（x±s）149.8±60.4、77.5±31.4、38.9±20.3,各期相互比较,差异有显著性P＜0.05。（3）观察组术前、术后一周,手术一周加服药一月后EMAb和CA125在血清中水平呈正相关.相关系数分别为r=0.91、r＝0.85、r＝0.75。结论:血清中EMAb及CA125水平可作为EMT诊断及治疗效果评价的一项有效的辅助指标。

Object: To investigate the clinical value of endometrial antibodies (EMAb) and CA125 in serum to the diaguosis and therapy of endometriosis. Method : To delermine prospectively both of EMAb and CA125 in serum of 20 cases of endometriosis diagnosed by operation and pathologic examination (observe group) and EMAb in 74 cases (control group Ⅰ ) or CA125 in 22 cases (control group Ⅱ) in whom endmetriosis were expelled. In the observe group still further analysed the variation of EMAb and CA125 in serum during different stages of preoperation, one week after operation and a month after taking Danazol bigining from one week after operation. Result (1) The conceutration of EMAb in serum in observe group (16. 6 ± 2. 3) was significantly higber than control group Ⅰ (1. 1 ±1. 0), P <0. 001, and CA125 in observe group (149. 8 ± 60. 4) was higher significautly than contro1 groupⅡ (29. 6 ±7. 1) .P<0. 01. The sensitivity to diaguosis in bothe of EMAb AND CA125 were 100%, aud their speciality was 91. 9% and 81. 8% respectively. (2) In observe group during different stages the EMAb in serum was 16. 6 ± 2. 3, 7. 9 ± 2. 3, 5. 5 ± 2. 2, and CA125 in serum was 149. 8 ±60. 4, 77. 5 ±31. 4, 38. 9 ± 20. 3 respectively.The value in different stnges had significant difference (P<0. 05). (3) And there were perfect positive correlation while analysed EMAb and CA125 in serum during those differeut stages. Conclusionf The concentration of EMAb aud CA125 in serum can be dealt with as an valuable index in diaguosis and therapeulic comment of endometriosis.
