关节镜下早期健侧腘绳肌腱重建前交叉韧带治疗膝关节脱位

Early arthroscopic reconstruction of the anterior cruciate ligament using uninjured side hamstring tendon autografts to treat traumatic dislocation of knee joint

目的探讨关节镜下早期应用健侧腘绳肌腱重建断裂的前交叉韧带（ACL）,治疗创伤性膝关节脱位的临床疗效。方法2006年12月-2009年1月,收治5例膝关节脱位患者,早期（伤后3周内）采用关节镜下应用健侧股薄肌、半腱肌腱重建ACL,评估患者术后并发症,膝关节活动范围。结果术后患者均获随访6-25个月,平均17个月,3例出现膝关节轻微僵硬,5例均不需要辅助能独立行走,2例后抽屉试验Ⅰ度阳性,1例腓总神经损伤运动感觉恢复良好。Lysholm评分（86±5）,关节活动范围（105±8.2）。结论关节镜下早期健侧腘绳肌腱重建前交叉韧带治疗膝关节脱位是一种有效的方法。

Objective The purpose of this study was to evaluate the clinical results of early arthroscopic reconstruction of the anterior cruciate ligament（ACL） using uninjured side hamstring tendon autografts to treat traumatic dislocation of knee joint.Methods We performed early arthroscopic reconstruction in 5 cases（5 males） of traumatic dislocation of knee joint,the semitendinosus tendon and gracilis tendon from the uninjured leg were used to reconstruct the ACL.Evaluate patients＇ postoperative complications and activity scope of the traumatic knee joint.Results Mean follow-up time was 17 months（range,6~25 months）.According to the last IKDC evaluation,the results were graded as normal in 2 patients,nearly normal in 3 patients,3 cases had minor knee stiffness,all patients could independently walk without aid,2 cases posterior drawer test were I degree positive,1 case with peroneal nerve injury recovered finely in movement and sensitivity.Lysholm knee score were（86±5）,activity score were（105±8.2）.Conclusion Early arthroscopic reconstruction of the anterior cruciate ligament（ACL）using unaffected side hamstring tendon autografts to treat traumatic dislocation of knee joint is effective method.
